Plan:
Measure the magnitude of the problem within academic hospital medicine, and, based on the type, source and magnitude of discrimination, identify appropriate policies and strategies to address the problem.  

Design:
Conduct a brief, anonymous, cross-sectional, electronic survey of academic hospital medicine faculty in the U.S.

Hi Renuka - this is an interesting and important idea. The topic is a sensitive one, obviously - how would we increase response rates? Also, is this questoinnaire based on any previous tools or scales? 

Hi Andy, thank you for your interest in this topic.

We plan to increase the response rate by making the survey easy, interesting and safe. We will use REDCap to distribute the survey, send reminders, and collect responses from the surveys. Participant tracking will be separate from the participant responses and there will be no link between the participant tracking and the responses.

There has been no published/ established tools or scales that are gold standard and hence we made this questoinnaire.

I have seen several published papers about racial, ethnic, and sexual discrimination among medical students and academic faculty. Here's one example by a colleague: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1492150/ It would be ideal to build off published work rather than using a new survey. 

A few other suggestions about content:

- Consider asking whether born in the US since approx 20-25% of hospitalists (or physicians in general) are not US-born and may expience more discrimination

- I would recommend broadening the response options for gender (at least include "other" and allow respondents to specify, though more specific options are also recommended. See https://www.hrc.org/resources/collecting-transgender-inclusive-gender-da...)

- If a physician experienced discrimination from their academic department or school, they may not be sure how to answer with regard to "someone in the hospital". Would you like to specify a hospital employee, and maybe have a separate question about an academic colleague or supervisor?

- Participants may be reluctant to provide their institution, as questions concern leaving their employment

PROTOCOL

Background:

Discrimination is an unfortunate but common occurrence in the work place today. Academic medical centers are not immune to this either. At all levels of training; medical students, residents, and provider levels; nursing, midlevel providers there has been documented discrimination in the workplace. A meta-analysis preformed (Fnais, N., et al. (2014). "Harassment and discrimination in medical training: a systematic review and meta-analysis." Acad Med 89(5): 817-827) in 2011 of studies documenting discrimination among all levels of health care providers (HCP) had shown verbal abuse 63%, gender discrimination at 54%, and racial discrimination at 24%.  In 2011, the Institute of Medicine released a report calling for the inclusion of questions around sexual orientation to be included in national surveys. More data is needed to understand how sexual orientation might contribute to stigma, discrimination and violence especially so that we can improve culture competency

My own personal experience with discrimination happened a few years ago. I was belittled as clinician woman of color by a well-educated middle aged patient. I continued to provide excellent care to this patient while keeping my feelings to myself as I had no place to turn, no one to help me deal with this situation. Currently there are no studies, surveying how health care providers deal with racism/discrimination from patients/families while they continue to care for these people.

Aim/ objectives:

Primary:

1. Understand the magnitude of discrimination for the hospitalists in the work place

2. Are there resources/ procedures in place for the hospitalists to deal with discrimination?

Secondary:

1. Blue States with Republican leadership experience more discrimination against hospitalists

2. Institutions with women leaders likely to have more support and policies in place

Impact: Academic medical centers are not immune from discrimination that is being reported in numerous sectors of the economy

Study design: We will conduct a brief, anonymous, cross-sectional, electronic survey of academic hospitalists’ nationwide.

The issue of discrimination against health care workers is a growing concern. Our survey aims to address these issues and create a culture that supports clinicians. Surveys are anonymous. Our study has minimal to no risk to the subjects. The surveys are voluntary. The benefit of conducting this survey will help us lay the foundation for making novel hospital policies and protocols. It will help us discuss possible interventions and responsibilities of clinicians and hospital leadership in taking care of patients and family members who exhibit racism and discrimination.

Data Collection: We will be collecting survey responses using REDCap, which is an IRB approved web-based system for distributing and collecting survey data. At the end of the study, data will be downloaded from REDCap on to a secure password protected local server at Weill Cornell Medicine. The PIs, will be the only ones that have access to the data.

Analysis Plan: Descriptive statistics will first be calculated for all questions and sociodemographic information. Differences between the different types of discrimination will be assessed by a Fisher’s exact test or a Chi-square test where appropriate. Additional analyses will be completed for data collected for secondary outcomes. Unless otherwise specified, all statistical tests will be two sided and the alpha will be set at 0.05 for consideration of statistical significance.

If ten or more institutions agree to distribute our survey, we will use random effect models to control for differences between institutions. These models will help describe variation and test if variation at different sites is significant.

Thank you for your comments. I reviewed the papers. They had a 177 item survey. They were very granular in their questions which made their survey lengthy. I think our survey reaches various points that the paper made.
