Mount Zion Health Fund

Overview

Mount Zion Health Fund reflects the Jewish values and legacy of Mount Zion Hospital by supporting programs and research that improve the physical, emotional, and spiritual health of vulnerable populations in San Francisco. Mount Zion Health Fund is committed to advancing the health of the underserved who have limited access to quality health services because of inability to pay, health status, race, religion, language, gender identity, or sexual orientation.

Eligibility

Proposed projects must originate at, connect with, or relate to the UCSF Mount Zion Campus. All UCSF health professionals are eligible to apply, as are local nonprofit or municipal agencies that provide services for underserved communities in San Francisco and have an identified UCSF Mount Zion Campus partner. Community partnerships are strongly encouraged. (Please note: we are not seeking to fund partnerships that are based on a transactional [e.g., vendor] relationship.)

Review Criteria

Proposed projects are required to:

  1. Explicitly benefit an underserved or vulnerable community.
  2. Identify and address unmet health-related need(s) in San Francisco.
  3. Originate at, connect with, or relate to the UCSF Mount Zion Campus.
  4. Embody one or more of MZHF’s core Jewish values.
  5. Include a plan for evaluation with both quantitative and qualitative outcomes.
  6. Demonstrate how the needs and perspectives of the intended beneficiaries have been integrated into the plan.

Priority will be given to proposed projects that:

  • Address health equity or other compelling San Francisco or UCSF Mount Zion healthcare priorities
  • Demonstrate an innovative understanding of or solution to a compelling need 
  • Create a model, mechanism or data others can employ
  • Can be successfully completed within the proposed time and budget
  • Equitably distribute grant resources
  • If intended as an ongoing project, articulate a plan for financial sustainability

 

Participation in Forum: UCSF Open Proposals visitors do not need an account to read proposals and read comments. An Open Proposals account is necessary to submit a proposal, submit comments, or to receive Forum notification updates. UCSF users may set up their own account directly. Non-UCSF users: please email karenst@sfjcf.org to request that an account be created for you. We will use your email as your user name and you will receive a one-time login which will allow you to create your account with a password. This step is necessary to prevent bot-generated spam comments.

Phase 1 - Template and Submission Instructions (Proposal Concept)

Please copy #1-6 prompts into the "Project Description" box below and respond to each prompt; applicants with a community partner should include the partner's contact information also.

  1. Applicant/s name; title; UCSF Mount Zion academic affiliation; Community Partner affiliation, if partner (note: we are not seeking to fund partnerships that are based on a transactional [e.g., vendor] relationship)

  2. Primary contact information: address, email, phone 

  3. Project Title

  4. Project start date and duration (earliest start date is December 1, 2023)

  5. Estimate of total budget required for project; total amount requested from MZHF; total amount per year if multiyear; if partner, provide total amount for each partner

  6. Brief Project Description (1-2 pages, including the following elements):
    • List of Goals

    • Summary of healthcare-related needs being addressed

    • Name(s), title(s), agency/department/division(s) of individuals who will lead the project (if partner, include Community Partner leader information), brief description of their individual roles, including brief background information relevant to ability to accomplish the project activities

    • Note which of the six MZHF values the project embodies

Phase 2 - Template and Submission Instructions (Full Proposal)

If invited to continue to Full Proposal Development (Phase 2, August 30 - October 2), please provide the following more detailed additional information. Please retain Phase 1 information, copy and append these prompts #1-7, and respond to each prompt.

  1. Specific aims

  2. Anticipated benefit for underserved or vulnerable communities in San Francisco

  3. How the project addresses UCSF Mount Zion priorities and compelling San Francisco healthcare needs

  4. Description of partnership (or the intentions to build toward meaningful engagement with community partner), including type, history and duration

  5. How the community partner's experience and expertise was integrated into proposal development

  6. Roles and responsibilities of each partner (Please note: we are not seeking to fund partnerships that are based on a transactional [e.g., vendor] relationship.)

  7. Requested grant period (e.g., start date of December 1, 2023 or later and duration of project)

  8. Upload Budget pdf in your own format, to include: total proposed project budget; total amount requested from MZHF; for 1-3 years with partner: amount per year for each partner. Include justification and line-item breakdown of revenue and expenses, including individual project team members' percent effort

  9. If project includes community partner/s, also upload the following for each community partner:

    • Current year total agency budget (specify budget dates), with line item breakdown of revenue and expenses and staff FTE commitments
    • Board of Directors List

 

Timeline

  • July 24 - August 24, 2023: Proposal Concept Submission (Phase 1)
  • August 30 - October 2, 2023: Optimization/Full Proposal Submission (Phase 2) - By Invitation
