Endoscopy Reprocessing Expansion Quality Initiative

OPG Proposal Status: 

The Endoscopy GI Techs have a longstanding history of providing high quality care and maintenance of endoscopes as measured by low repair costs and the absence of infections transmitted to our patients.  We currently have approximately 15 GI techs who reprocess scopes at all three campuses during business hours.  As a result of the CRE outbreak at UCLA, we have expanded the responsibilities of the gi techs to provide 24/7 coverage at all three campuses to ensure the same standard of care is practiced around the clock.

Over the past few years, we have had a movement in critical care to reduce unnecessary anterior-posterior (AP) portable chest x-rays (CXRs), but our system has not caught up to our intention.

Reducing repeat colonoscopy by improving the quality of bowel preparation

OPG Proposal Status: 

Unlike other procedures, colonoscopies require patient participation to clean the colon and poor  prep means repeat procedures, repeat sedation and missed polyps.

Cardiology Daily Weights

OPG Proposal Status: 

Daily weights on the cardiology service are sometimes incongruent with patient intake/output (I/Os) or physical exam which leads to confusion in clinical decision making. Several factors have been identified by nursing, patient care assistants and housestaff that might lead to inaccurate reporting of daily weights. Targeting these to improve reliability of daily weights should optimize inpatient diuresis (particularly in cases of unreliable I/Os and difficult physical exams) leading to reduced incidence of under and over-diuresis. Downstream this could potentially impact length of stay as well as readmission for heart failure patients and thus result in cost savings for patients and for UCSF.

Inpatient Diabetes Part Two. System wide Implementation of Self Adjusting Subcutaneous Insulin Algorithm

OPG Proposal Status: 

NOTE:  We have submitted two inpatient diabetes proposals.  While each can be looked at independently, we believe both can be implemented concurrently.

Background and Significance:

Inpatient Diabetes Part One. Automating Inpatient assessment of diabetes knowledge and education needs and triaging education/dietary resources to those with deficits.

Background and Significance:

Antimicrobial Self-Stewardship: Leveraging IT solutions to combat antibiotic misuse

OPG Proposal Status: 

We are proposing the development of two Apex IT solutions that will facilitate both antimicrobial self-stewardship (antimicrobial clock) and antimicrobial audits from consulting services like infectious disease and pharmacy (antimicrobial report card).
